Expertise and Research InterestsIn the last five years, the paediatric respiratory group's research has focused on factors that affect the effective management of respiratory illness in children. One strand, building on our experience of electronic monitoring of inhaled medication use, has combined objective electronic monitoring with more qualitative approaches to explore patterns of parental medication use in preschool children (National Asthma Campaign Award No. 354, £99,453), and the impact of an educational intervention and its effect on compliance. A second has been the impact of education on disease management. We were the first group to establish that nurse-led home-management training could very substantially reduce subsequent hospitalisations. Recognising the large effects of properly constructed educational interventions, a study is currently investigating the effects of a targeted package in high-risk teenagers with severe asthma (funded by NHS R & D NHS Programme on Asthma Management (AM2/07/023), £117,544). It also led to the award of a National Asthma Campaign five year Senior Research Fellowship to P. Madge to investigate barriers to the delivery of care in children with asthma. The first fruits of this are a recent award to investigate the importance of the behavioural concept of self-efficacy in children with acute asthma (Scottish Office Department of Health, (K/OPR/15/11/F11), £12,609).
Work with Human Nutrition (Dr. John Reilly) has resulted in studies exploring energy expenditure in respiratory disease. A developing collaboration with Human Nutrition and Exercise Science (Dr. John Grant) has explored direct methods of measuring activity in preschool children as well the impact of activity on body composition as factors influencing activity in preschool children, physical activity, and energy expenditure in preschool children (Sport Aiding Medical Research for Kids (SPARKS), £88,250). Other ExpertiseFlexible bronchoscopy in children Future ResearchWe have been interested in the idea of therapeutic concordance and have exploring factors that promote or inhibit therapeutic concordance between doctor and families. We are particularly interested in those factors that encourage successful self-regulation and promote, or inhibit, compliance. We plan to continue investigate the design and delivery of interventions, such as computer based ducation programmes, that may promote self-regulation as well as developing a better understanding of the barriers, both patient-related and doctor/nurse related, that inhibit such skills developing.
We plan to continue studies on physical activity in early life and its relation to body composition and cardio-respiratory health and fitness in pre-school children.
