Expertise and Research Interests

Clinical:
Oral medicine (registered specialist)
Surgical dentistry (registered specialist)

Research:
My prinicipal areas of research are:

1. Minimally invasive management of salivary calculi
Sialolithiasis accounts for more than 50% of cases of major salivary gland disease and is therefore the most common cause of acute and chronic salivary infections. Until now the treatment of choice for calculi that lie in the proximal duct or gland has been sialadenectomy despite its attendant risks. An alternative, minimally invasive approach based on shock wave lithotripsy, radiologically guided basket retrieval, endoscopically guided surgery and conservative surgical intervention is currently being pioneered in some European centres. An important principle that underpins this development is that the secretory function of the affected gland can regenerate after removal of the obstruction. It is possible that, with time, this approach will replace sialoadenectomy as the primary treatment option.

2. Management of oral mucosal disease.
Traditionally the management of such conditions has been based on consensus. However, with the move towards evidence based practice there is a need for objective assessments of outcome to facilitate prospective randomised clinical trials. We are currently developing clinical scoring systems for a number of conditions including lichen planus, mucous membrane pemphigoid, pemphigus vulgaris and oro-facial granulomatosis.

3. Managment of oro-facial granulomatosis
Oro-facial granulomatosis (OFG) is a chronic inflammatory disorder presenting characteristically with lip swelling. Most cases of OFG occur as a separate clinical entity but a proportion present in association with established CrohnÂ's disease of the small or large intestine. We have shown that in a large proportion, particularly younger patients, the oral inflammation may form part of a generalised, discrete granulomatous enteritis. We are currently investigating the role of dietary restriction inthe managment of such patients.
