Since 1994, colleagues at Group Health Cooperative and I have conducted a project entitled Breast Cancer Surveillance in a Defined Population. They have developed a comprehensive system for collection of information on breast cancer risk factors, mammography occurrence and results, biopsy findings, pathology results and specimens, and breast cancer incidence in the population of Group Health women. This system is used to answer a range of research questions concerning factors that increase a woman?s risk of breast cancer or increase the risk of a cancer being missed by screening. Findings so far include that increased mammographic breast density is the major cause of cancers missed by mammography (false negative mammograms), with a six-fold risk for women with the most breast density. Fast tumor growth (measured by Ki-67 on tumor specimens), use of hormone replacement therapy and poor quality mammography also were found to contribute to false negatives exams. They also found that among pre-menopausal women, breast density isless during the follicular phase of a woman?s cycle; therefore screening during that time might be a way to improve screening effectiveness for pre-menopausal women. This is under further investigation.

In 1999, with Ruth Paterson and colleagues, I received funding for the VITAL (VITamins And Lifestyle) study. This was a prospective study of men and women in Washington State, to determine whether use of vitamin and mineral supplements is associated with cancer risk. Over 76,000 men and women entered the cohort by completing the baseline questionnaire and, of those, over 40,000 provided DNA through self-collection of cheek cells. This study will be used to understand whether dietary supplements, which are among the most commonly used drugs in the US, are associated with decreased risk of cancer, have no impact, or have an adverse effect, and to investigate the interactions of genetic factors with nutrients in cancer susceptibility.
