Expertise and Research Interests

Clinical, molecular, and genetic epidemiology is my main area of expertise. Since year 2000, my laboratory has focused on conducting translational research that spans from the characterization of clinical phenotypes using standard epidemiologic methods to the identification of genetic variants that predispose to the development of these phenotypes, and translating these findings into potential biomarkers that can be used to identify individuals at risk for developing these phenotypes. Our genetic epidemiologic studies are based upon high-throughput whole gene screens of candidate pathways using a tagSNP approach, and in the near future, will expand to whole genome screens. Specific phenotypes occurring after hematopoietic stem cell transplantation that we are currently studying include airflow obstruction (bronchiolitis obliterans), idiopathic pneumonia syndrome, Gram-negative bacteremia, invasive fungal infection, cytomegalovirus infection, and community acquired respiratory viral infection. Our research has also recently expanded to include tobacco related respiratory diseases. Specifically, we have taken similar research approaches toward identifying genetic and molecular markers that will identify individuals who are at risk for developing chronic obstructive pulmonary disease and lung cancer.

These studies have led to molecular genetic studies in the laboratory that include complete sequencing of candidate genes for discovery novel functional mutations, transfection and mutagenesis studies to determine the function of novel mutations, and molecular assays to characterize the intermediate phenotypes resulting from these mutations.

Other Expertise

Clinically, I am an expert in the management of noninfectious pulmonary complications, the most common of which are hematopoietic stem cell transplant related airflow obstruction, otherwise known as bronchiolitis obliterans syndrome (BOS), and bronchiolitis obliterans organizing pneumonia (BOOP).

I am also the director of the Lung Cancer Early Detection and Prevention Clinic at the Seattle Cancer Care Alliance. This is a clinic dedicated to the health of current and former smokers, focusing on the prevention and early detection of the most common tobacco related lung diseases such as COPD, emphysema, chronic bronchitis, and lung cancer.

Industrial Relevance

I have developed and validated the "PAM score" (Pretransplant Assessment of Mortality Score), which is accessible as a simple caluclator by all clinicians via the website "http://cdsweb.fhcrc.org/pam/". This score provides information with regard to the mortality risk a patient is likely to experience if he or she undergoes an allogeneic hematopoietic stem cell transplant (HCT). The scoring algorithm was developed and validated using clinical data from over 2800 allogeneic transplants performed from 1990 to 2002 at the Seattle Cancer Care Alliance/Fred Hutchinson Cancer Research Center. Five validation cohorts from our Center were used to assess the performance of this predictor tool: 1990 to1997 cohort, 1997 to 2002 cohort, all chronic myeloid leukemia patients, all acute myeloid leukemia patients, and all myelodysplastic syndrome patients. The analyses found that this tool can predict the mortality risk of a patient considering allogeneic HCT with reasonably high accuracy. The details of the methods and results are described in our March 2006 Annals of Internal Medicine publication.
