Expertise and Research Interests1. To examine medical decision making in ambulatory settings. I am studying the process of decision making, determining resources used in evaluating primary care complaints, the level of comfort that exists with uncertainty, and the influence of economic factors such as method of payment. I am using encounter-based medical information as recorded on a modified billing document, linking reason for encounter to lab studies ordered, ancillary services requested, and diagnosis. My objective is to describe the process of decision making for common complaints (or RFEs), and to evaluate the effectiveness of decision making and patient management strategies. A related interest is the development of a computer-based, research-oriented information system for a primary care office. We have designed and implemented a Macintosh-based system that integrates billing and research functions and performs quality assurance screening.
2. To determine the process involved in quality assurance in primary care and to link it to outcomes through explicit and implicit "quality" measurement.
3. To determine how changes in health care economics affect the patient/physician relationship.
