Expertise and Research Interests

Salivary Stones
A centre for the study and minimally invasive treatment of salivary of gland disease have been developed at Guy's Hospital. Our work has completely changed management of salivary stones. Approximately 3,000 patients each year in the UK who would normally have their glands removed can be treated successfully by minimally invasive means.

Salivary Tumours
My work has shown that the traditional invasive surgery for benign parotid tumours can be replaced by a much more limited procedure that is quicker and has less morbidity. Once again it is revolutionising the current practice of parotid surgery, moving away from a major to a minor procedure. We are the only centre in the UK developing this minimally invasive surgery for benign salivary tumours.

Head and Neck Cancer
Guy's Hospital has a long history in the practice of head and neck surgery. I have developed a comprehensive head and neck service which includes microvascular reconstruction. A head and neck database compiles patient details and an active research programme using PET detection of squamous cell carcinomas is currently being undertaken. The outcome achieved is a personal series of 287 mouth cancers above the South Thames Cancer Registry results (53% v 41% crude 5y survival).

Day Case Oral Surgery
This involves mainly removal of impacted third molar teeth and is one of the commonest procedures undertaken in the NHS. Our work has looked at efficient provision of this service and has investigated factors that predict for difficult cases and complication of surgery. We are also studying techniques to facilitate early treatment for damaged lingual nerves, the commonest serious complication.

Future Research

- The successful application of minimally invasive techniques to surgery in the jaws is dependent upon developing sophisticated imaging systems. This is because minimally invasive surgery is based on endoscopic technique that severely limits the surgeon's field of view. The application of endoscopic technology to facial reconstruction requires the surgeon to have a clear appreciation in 3D of the underlying facial skeleton. We have developed methods of accurately colocalising different image modalitiessuch as CT, MRI and PET scans to the face in real time. These computer images can be projected onto the patient to assist endoscopic surgery.

- In the field of cancer, we lead a major EORTC head and neck study on sentinel node biopsy. I have proposed a computer model to establish a national standards for treatment outcome.

- We are evaluating the use of minimally invasive techniques in the management of salivary stones in a multi institutional study. In the laboratory we are also investigating the effects of obstruction on salivary gland function and the factors responsible for regeneration of the gland.

- We are expanding the role of microendoscopy into breast surgery and developing new equipment which includes endoscope magnification of images (50x) and computer assisted imaging system.
